The electronic entry form has been malfunctioning since mid-August due as files and programs are being transferred to new web servers.
Attempts to "Submit" the form usually generated error messages, which obviously indicate that your update was unsuccesful. But recently, you may have received the correct "Thank You, Information received" message, even though the information does not actually get forwarded to me.
The electronic form has therefore been removed until it will function correctly. You can wait until the electronic form is back, or use the manual form.
